New to mods, PS4 can't turn any on? Help

I just got a new modded controller and can't seem to get any of the mods on. Please help! The led indicators dont turn on either!

Can you use R1 and L1 buttons in the game? The further troubleshooting steps will depend on your answer.

It looks like your controller needs to be sent back for an inspection. You can do so by logging in to your account and requesting RMA.
